SERVICE AGREEMENT: ECO LIVING ASSIST
1. PARTIES THIS AGREEMENT IS BETWEEN ECO LIVING ASSIST (ABN:
41606875030) AND THE PARTICIPANT NAMED IN THE ATTACHED SCHEDULE OF SUPPORTS/QUOTE.
2. PURPOSE OF AGREEMENT 
THE PURPOSE OF THIS AGREEMENT IS TO PROVIDE SMART HOME SAFETY
EQUIPMENT AND PRE-CONFIGURATION SERVICES. THESE SUPPORTS ARE
DESIGNED TO ASSIST THE PARTICIPANT IN MAINTAINING INDEPENDENCE AND SAFETY WITHIN THEIR HOME ENVIRONMENT.
3. SCOPE OF SERVICE
• INCLUSIONS: DELIVERY OF THE SMART HOME SAFETY KIT AND TECHNICAL
PRE-CONFIGURATION OF HARDWARE (E.G., TRAVEL ROUTER, SENSORS, AND
HUBS) TO ENSURE THE SYSTEM IS "PLUG & PLAY" UPON ARRIVAL.
• EXCLUSIONS: ECO LIVING ASSIST IS NOT AN INTERNET SERVICE PROVIDER
(ISP). WE ARE NOT RESPONSIBLE FOR EXISTING HOME INTERNET SPEEDS,
RELIABILITY, OR ISP OUTAGES. PHYSICAL INSTALLATION
(DRILLING/MOUNTING) IS NOT INCLUDED UNLESS SPECIFICALLY QUOTED AS A SEPARATE SERVICE.
4. PAYMENTS & NDIS COMPLIANCE
• INVOICES WILL BE SENT TO THE NOMINATED PLAN MGR OR PARTICIPANT 
• GOODS WILL BE DISPATCHED ONCE PAYMENT HAS BEEN CONFIRMED 
• MOST ASSISTIVE TECHNOLOGY IS GST-FREE; HOWEVER, IF GST APPLIES TO ANY COMPONENT, IT WILL BE CLEARLY NOTED ON THE INVOICE.
5. WARRANTY & LIABILITY
• HARDWARE: ALL DEVICES ARE COVERED BY STANDARD AUSTRALIAN
CONSUMER LAW WARRANTIES.
• SUPPORT: TECHNICAL SUPPORT IS PROVIDED FOR THE INITIAL SETUP OF THE PROVIDED KIT. ONGOING MAINTENANCE OF THE PARTICIPANT'S PERSONAL HOME NETWORK REMAINS THE RESPONSIBILITY OF THE PARTICIPANT.

AGREEMENT CONSENT:
BY PAYING THE ATTACHED QUOTE/INVOICE, THE PARTICIPANT (OR THEIR
REPRESENTATIVE) AGREES TO THE TERMS AND CONDITIONS OUTLINED IN THIS SERVICE AGREEMENT.
